Your First Visit
Welcome to the Knox County Community Health Center. Our hours of operation are Monday thru Friday 8:00 to 4:30pm. All visits are by appointment only. Appointments can be made by calling 309-344-2225.
Once your appointment is made, you will receive a packet in the mail or you can download the documents.
This packet contains all the material you will need to complete and bring to your appointment. The day of your appointment you will need to arrive 20 minutes early to ensure all paperwork is completed. It is during this time, that our Intake Specialist will determine where your income falls on our sliding scale. Our sliding scale system is a discount based on your household income and size. Currently, each visit will cost $10.00.
