WHO IS ELIGIBLE FOR SERVICES?
Knox County residents eligible to receive services at the Dental Clinic include:
Medicaid eligible children aged 3 years and older
INFORMATION TO BRING WITH YOU...
You MUST bring your current Medicaid card with you to each appointment!
The Health Department serves as an applicant assistance agency for Illinois AllKids insurance coverage. Ask us if your kids qualify!
ATTENTION PATIENTS!
Payment for services rendered must be made by either cash or check at the time of service.
