Family Case Management

Assists women and infants up to 18 months of age who are Medicaid-eligible or low income. A special emphasis is placed on assuring early prenatal care in an effort to promote the delivery of a healthy baby.

• Case management services include:
• Supportive personal contact
• Health education
• Assessment of needs
• Coordination of medical care
• Referral to other social service agencies
