Knox County residents eligible to receive services at the Community Health Clinic include individuals who are:
At or below 250% of poverty level; and
Do not have Medicare or health insurance of any kind

ATTENTION PATIENTS!
Payment for services rendered must be made by either cash or check at the time of service.
In order to be eligible for services offered by the Knox County Community Health Clinic, all clients must provide proof of Knox County residency and verification of household income at least once per year.
Income verification can be provided by:
the past year’s tax return; or
two current pay stubs.
Proof of residency can be provided by:
A current State ID; or
Recent mail indicating address.

Fees are assessed at the time of service and include the following:
