Behavioral Health Counselor
Job Description 
  • Function in a timely and appropriate manner as a team member on an integrated behavioral health team.
  • Provide behavioral and mental health services to patients; counsel individuals and groups regarding problems such as stress, substance abuse and family conflict, in order to modify behavior and /or improve personal, social and vocational adjustment.
  • Develop and implement individual treatment plans, specifying type, frequency, intensity and duration of therapy; discuss treatment of problems with clients.
  • Evaluate the effectiveness of counseling or treatments, and the accuracy and completeness of diagnoses, modify plans and diagnoses as necessary.
  • Identify psychological, emotional or behavioral issues and diagnose disorders, using information obtained from interviews, tests, records, consultations with primary care providers and reference materials.
  • Interact with clients to assist them in gaining insight, defining goals and planning action to achieve effective personal, social, educational and vocational development and adjustment.
  • Obtain and study medical, psychological, social and family histories by interviewing individuals, couples or families and by reviewing records.
  • Provide occupational, educational and other information to individuals so that they can make educational and vocational plans.
  • Track clinical symptoms over time using established measures; and cue changes in treatment when the patient is not improving.
  • Support medication therapy prescribed by PCP.

Director of Compliance and Performance Management
Job Description
  • Lead all quality improvement efforts and accomplish goals set forth in the Knox Community Health Center strategic plan.
  • Manage multiple improvement projects simultaneously.
  • Serves as Compliance Officer for the Knox County Health Department and Knox Community Health Center; is designated to overseeing the Quality Improvement/Quality Assurance program and functioning as an independent and objective resource that reviews and evaluates potential areas of quality, compliance, performance, vulnerability, and risk.
  • Monitors Quality Improvement efforts and Quality Assurance outcomes and develops/implements corrective action plans for resolution of problematic issues; provides general guidance on how to avoid or deal with similar situations in the future within the Health Department and Community Health Center.
  • Creates and implements the Health Department and Community Health Center Compliance Plan, ensuring management and employees comply with rules and regulations of State of Illinois, Federal, and other regulatory agencies, that Quality Improvement/Quality Assurance and other company policies and procedures are up to date and being followed, and that behavior in the organization meets standards of conduct.
  • Investigates, develops, and facilitates accreditation and certification activities for the Knox County Health Department and Knox Community Health Center; serving as a resource for information on processes; maintaining required documentation, and coordinating and disseminating information to all individuals involved with the processes.
  • Creates plans, monitoring implementation and reporting of Performance Management program for the Knox County Health Department that is inclusive of, but not limited to; Quality Improvement, Risk Management, Community Engagement, Workforce Development, Community Health Improvement, and Strategic Planning.

Registered Dental Hygienist
Job Description
  • Uses learned dental hygienist practice, theories, and principles to provide direct services to individuals, families, and community environment in the Community Health Center.
  • Performs a complete dental prophylactic service that includes an exam, cleaning, appropriate x-rays, fluoride treatment and oral hygiene instruction.
  • Administers approved various prevention treatments such as sealants to tooth surfaces.
  • Examines oral structures for evidence of disease and reports suspicious findings to the dentist.
  • Documents and records all procedures performed including date, services provided, clinical information and determines the time for the next visit.
  • Collects preliminary data such as necessary x-rays, full-mouth periodontal charting and oral cancer screening.
  • Reviews and updates the patient’s health history, dental history, and dental charting at each visit.
  • Prepares patient room by sterilizing hygiene and operative instruments, keeping the instruments sharpened, disinfecting the room and equipment after each patient, making sure that the room is stocked and placing barriers on equipment in accordance with OSHA guidelines.
  • Implements and follows the Dentist’s recommendations for the patient’s treatment plan.
  • Educates the patients as to the condition of their oral health, their option for treatment, consequences of treating or not treating and provides home care instructions including alternatives for diet.
  • Work with the public, exhibiting courteous behavior, non-judgmental attitude, being culturally aware and maintaining client confidentiality.
  • Renders soft tissue management such as scaling, root planning, incidental curettage, sub gingival irrigation, gingival massage, and application of prescribed mendicants.
  • Develops a personalized homecare program for all periodontal patients.
  • May assist in training new employees.
  • May perform basic office tasks such as filing, checking-in and checking-out patients in the computer, calling patients, scheduling appointments and other office duties in the Community Health Center.
  • Develops and maintains effective working relations with employees, patients, representative of government and community officials, service providers and the general public.

Job Description
  • Provide quality dental care services to eligible beneficiaries; to include disabled, and medically and/or psychologically compromised in the Community Health Center.
  • Perform oral examination of patients, analyze dental health problems, and develop appropriate treatment plans, in a manner which meets patient care needs as well as industry and State standards.
  • Provide treatment, within the scope of privileges and Department capacity, exercising sound judgment in determining scope of authority and level of ability of self and subordinates.
  • Refer patients which are beyond the scope of the Community Health Center Dental Clinic and staff in a manner consistent with established policies and procedures.
  • Maintain an understanding of the Community Health Center dental equipment and provide supervisory and advisory counsel to other staff members ensuring proper function and maintenance.

Qualifications

Graduate of a United States accredited school of dentistry; and

Licensure as a Doctor of Dental Surgery (D.D.S.) or Doctor of Dental Medicine (D.M.D.) by the State of Illinois; able to apply for a current DEA license; and

Dental Assistant
Job Description

  • Prepare Community Health Center dental patients for diagnosis and treatment by the Dentist, as well as assist Public Health Dentist during diagnosis and treatment of patients.
  • Conduct coronal polishing, and take and develop x-rays for Community Health Center patients when requested.
  • Monitor inventory of supplies for Community Health Center, assist in re-ordering, and ensure operatories are appropriately stocked with dental supplies at all times.
  • Utilize knowledge of appropriate referral systems and act as a facilitator to complete dental related client referrals.
  • Complete required documentation for patient charting complying with all appropriate policies and procedures, as required by the Department’s HIPAA Privacy Standards and billing procedures.
  • Provide assistance in front desk functions, as appropriate, to include confirmation of dental appointments, filing charts, and scheduling appointments
