Payment Information and Sliding Fee Scale


We accept Medicare, Medicaid (including MCO’s) and private insurance. If you have a medical card, please make sure your Primary Care Provider (PCP) is listed as the Knox Community Health Center before your appointment.

KCHC accepts cash, check, Visa, Mastercard, Discover, and debit card payments.

Patients will receive a billing statement from the health center. Payments can be mailed or brought to the health center for processing.

Fees that are the patient’s responsibility are due at the time of the service.


Sliding Fee Scale

Sliding fee scale is available for those who qualify. Proper documentation must be provided to qualify:

  • Photo ID
  • Any current insurance including Medicare, Medicaid (MCO) or private insurance if applicable
  • 30 days proof of income for the entire household. Proof of income includes the following:
    • Paycheck stubs
    • W-2 tax forms
    • Disability benefits
    • Child support
    • Social Security
    • Retirement income

Patients who state they do not have any income will need to complete a zero-income affidavit form.

If a patient is unable to or refuses to provide the required documents, then that patient will be responsible for the full bill.

A patient’s eligibility for the Sliding Fee Scale will be reviewed each March and the patient must present updated information to requalify.

The current Sliding Fee Scale is based on the most recent Federal Poverty Level guidelines and is approved by the KCHC Governance Board annually affective on March 1st.

Sliding fee scale qualifications and benefits can be reevaluated at anytime to reflect changes in income.

Marketplace Navigator Program

The KCHC Navigator is trained and available to assist you with finding health care coverage. If you have had a recent life changing event and have lost your insurance, we may be able to help you. Come see our licensed Navigator to sign up for Marketplace insurance or Medicaid.

Certified Application Counselor

The KCHC has a Certified Application Counselor (CAC) trained and ready to help you explore health coverage options through the Marketplace. The CAC will help you complete eligibility and enrollment forms.

Open Enrollment Facts

  • Open enrollment is available November 1, 2022 through December 15, 2022.
  • Enrollment starts with HealthCare.gov. If your state uses its own website HealthCare.gov will guide you there. The plans available through this website are ACA plans which are comprehensive plans that provide coverage at an affordable price.
  • Healthcare.gov plans must include benefits such as mental health care, maternity care, prescription drug coverage and hospitalization.
  • Most marketplace shoppers will qualify for a discount based on their income.
  • Enroll online at HealthCare.gov, over the phone by calling 1-800-318-2596 or come in to see us to get started.
