Bus Route: The Knox Community Health Center is located on the red bus line. The bus stops at the health department once per hour, at fifteen minutes past, between 7:15am-5:15pm. More information about bus routes is available through the city of Galesburg.
Parking: Please park in the front of the building, the entrance to the Knox Community Health Center is the far left door (west side).
Appointments
Appointments are preferred, but walk-ins for medical are also welcome. Same day appointments may be available for medical and dental.
Keeping your appointment with KCHC is important but we understand that sometimes things occur and appointments need to be changed. If you need to reschedule your appointment, please call us as soon as possible so that we can get you back on the schedule for a date that works better for you.
Patient Portal
The Knox Community Health Center offers the Patient Portal. The Patient Portal keeps your health information at your fingertips allowing you easy access to your health information.
The portal allows you to:
- Send and receive secure messages between you and your provider
- View, download or print your Visit Summary
- Request appointments
- Get appointment reminders
- Update your personal information
