Individuals under the age of 18 are required to be accompanied by a parent or legal guardian. A parental consent form must be signed before the vaccine can be administered.
Please remember to bring your vaccine card for second, third, or booster doses.

Get Vaccinated and Stay Up To Date On Your Vaccines
You are up to date with your COVID-19 vaccines if you have completed a COVID-19 vaccine primary series and received the most recent booster dose recommended for you by the CDC. Use CDC's COVID-19 booster tool to learn if and when you can get boosters to stay up to date with your COVID-19 vaccines. Stay up to date by getting all recommended boosters when you are eligible.
**Note that booster vaccines are not recommended for everyone at this time.
How to Access Vaccine Records
If you need a copy of your vaccine record you can access it through IDPH's immunization portal, Vax Verify. Vax Verify now includes SMART™ Health Card, an easy way to share your COVID-19 vaccination status. The SMART™ Health Card allows users to download a QR code which individuals can use to confirm their COVID-19 vaccination status.
